Look at your motives for doing this.

 Use your knowledge of her to your advantage, especially in what appeals to her.
 So ditch the pre written card and write in your own words how you truly feel about her, and either make or buy her something that you know that she will really appreciate.
  Maybe she has started to move on, or maybe she has already met someone.
Don't give up working towards your goal of winning her back what ever the outcome may be.
Keep being thoughtful and considerate to her.
If she has started dating things may not work out with this new guy or she may realize you really are the nice caring person she has always loved and come back to you.
