How to Make a Sign-up Form in PowerPoint

    • 1). Open a new PowerPoint document. Decide whether the sign-up form will be portrait or landscape orientation. The slides are automatically displayed in landscape. For portrait orientation, click on "File" then "Page Setup." Click the "Portrait" radio button.

    • 2). Click the "Insert" tab on the PowerPoint Ribbon, then click the "Table" button on the left to open a drop-down window that displays a grid structure.

    • 3). Use the displayed grid to select the number of rows and columns for the table based on what you need for the sign-up form. A potluck sign-up form, for example, might have two columns: one for people's names and one for food items. If you want to allow space for nine people to sign up, the form will need 10 rows: one for the column titles ("Name" and "Food Item") and nine for people's details.

    • 4). Add column labels by clicking inside the top box of each column. Make sure your column labels are clear, so that people know exactly what information you need. Set the column titles apart from the rest of the form by applying bold formatting or other highlighting. To apply bold formatting, select the titles, hold down the "Ctrl" key and press the letter "B."

    • 5). Add a title to the form by inserting a text box. Click the "Insert" tab, then "Text Box." Click and drag on the slide, above the table, to create the text box. Click inside the text box and type the title name, for example, "Field Trip Volunteers Needed" or "Company Potluck."

    • 6). Modify the size and position of the title text box. To change its size: click the text box to select it, move the mouse pointer over one of its edges or corners until a two-way arrow appears, then click and drag the edge or corner. If you drag an edge, the box will size in one lateral direction only. If you drag a corner, it can be sized in both directions at once. To move the title: click the text box to select it, hold the mouse pointer over the middle of the box until a four-way arrow appears, then click and drag the box to its new location.

    • 7). Add pictures or graphics to provide visual appeal. This is an optional step, however, graphics can also help identify the purpose of the sign-up form. To add pictures or graphics, click "Insert" on the toolbar, then choose "Picture" from the drop-down menu. Choose "Clip Art" to add graphics that are available within PowerPoint. Choose "From File" to insert pictures stored on the computer itself.
