Other benefits include $35 co-pays for outpatient rehabilitation services including speech therapy, physical therapy and occupational therapy. Participants are allowed up to 30 visits per year.

      The plan also offers coverage for family planning, mental health, lab work, X-rays and prescription drug benefits.
