Correct Way to Fill Out a Deposit Slip

If you don’t have a pre-printed deposit slip and must use the blank slips the bank provides, you will need to start by printing your name, address, and phone number in the spaces provided (normally on the left side of the deposit slip). Next, enter your bank account number. There may be a box to check to indicate if the deposit is for a checking or savings account.
Enter the date and sign the deposit slip in the spaces provided. Make sure each check is properly endorsed. If you are not getting any cash back, you should write “For Deposit Only” on the back of each check.
Recording Deposit Items
On the top right of the deposit slip, enter the total amount of any cash, including coins, you are depositing. List the amount of each check in the spaces provided. If you have more checks to deposit than spaces on the front of the check, turn the deposit slip over and write the amounts of each remaining check. Add up the amounts of the checks listed on the back of the deposit slip and write this total in the space provided on the front. Then add up the amounts listed on the front of the deposit slip and write the figure in the space labeled “Subtotal.” Enter the amount of any cash you are getting (usually this space is labeled “Less Cash Received). Subtract the cash you are getting from the subtotal and enter the remainder in the box labeled Total or Total Deposit.
